Chronic Care Management and Care Plan Oversight
Medicare’s Chronic Care Management (CCM) program allows providers to bill for time they spend on non-face-to-face care coordination services for Medicare patients who have two or more chronic conditions, such as diabetes, hypertension, COPD and heart disease. Additionally, Medicare’s Care Plan Oversight (CPO) allows a provider to bill for supervision of a patient who is receiving complex and/or multidisciplinary care provided by a participating home health agency or Medicare-approved hospice.
